Özet

Sarcoidosis is a systemic granulomatous disease affecting all tissues and organs in the body. The anaesthesiologist must be aware of the patient's general status and must search for evidence of pulmonary and cardiac abnormalities in the preoperative period. In addition, close hemodynamic and respiratory monitoring should be used during the perioperative period. These patients may present many potential problems for the anaesthesiologists. In this case report, we show the succesful anaesthetic management of a patient with sareoidosis under spinal anaesthesia. A 68-year-old female with sareoidosis was scheduled for a total abdominal hysterectomy. She had no other past medical history except hypertension. Perioperative monitoring included ECG, noninvasive blood pressure and peripheral oxygen saturation. We choose spinal anaesthesia over general anaesthesia because of the decreased respiratory and cardiac changes. After intravenous administration of 10 mL kg-1 0.9 % NaCl solution, the patient was placed in the left lateral position, and 22.5 mg of 7.5 % ropivacaine was injected into subarachnoid space via a 25 gauge spinal needle. The hemodynamic and respiratory status was stable and no complications were noted during anaesthesia. In conclusion, we think that spinal anaesthesia is safer than general anaesthesia for patients with sarcoidosis.
