Root canal treatment of a tooth with radix entomolaris: a case report

Özet

Usually, first mandibular molars have one mesial and distal root but in some cases there are anatomical variations wherein the number of roots and root canals vary. Presence of an additional lingual root distally in mandibular molars is called radix entomolaris (RE). Appropriate diagnosis is must before starting with root canal procedure in these teeth to ensure successful treatment outcome. To emphasize the importance of accurate diagnosis and appropriate treatment planning in cases were rare root morphologies, such as RE, may affect the success of endodontic treatment. 19-year-old male patient presented to the clinic with a complaint of pain. The patient’s medical history was unremarkable for any systemic diseases. A diagnosis of irreversible pulpitis was made for tooth number 36. Preoperative periapical radiograph revealed the presence of a radix entomolaris in tooth number 36. After access cavity preparation, the presence of three roots and four root canals was confirmed. The canals were instrumented and subsequently obturated with gutta-percha and AH Plus sealer. The tooth is currently asymptomatic, and follow-up is ongoing. Recognizing anatomical variations like radix entomolaris is crucial for the success of endodontic treatment. With proper imaging, careful canal exploration, and correct instrumentation, successful outcomes can be achieved in these cases.
