Özet

Giriş ve Amaç: Lomber disk protezi; füzyon cerrahisine alternatif olarak geliştirilen bir yöntemdir. Komşu segment hastalığı füzyon cerrahisinin en büyük sorunlarından biri olması nedeniyle alternatif yöntemler her zaman denenmiştir. Biz de bu çalışmada lombar disk protezlerini ve komplikasyonlarını literatür eşliğinde tartıştık.

Gereç ve Yöntem: 38 yaşında kadın hasta, 6-7 aydır mevcut olan ve son zamanlarda şiddetlenen sağ kalça ve uyluk anterolateralinde ağrı şikayetiyle kliniğimize başvurmuş ve L4-L5 seviyesinde kanala idante disk nedeniyle diskektomi uygulanıp, total intervertebral disk protezi yerleştirilmiş-

tir. Takiplerinde 1 yıl sonrasında şiddetli sol bacak ağrısı şikayetiyle gelen hastanın tetkiklerinde protezin kanala doğru disloke olduğu gözlendi. Reoperasyon ile materyal çıkarıldı.

Bulgular: Posterior yolla takılan disk protezleri anterior ve ekstreme lateral yola göre daha az komplikasyonlara neden olmaktadır. Literatüre bakıldığında lombar disk protezleri hakkındaki kafa karışıklığı halen devam etmektedir. Ancak anterior yaklaşımın üstünlükleri konusunda çok sayıda yayın bulunmaktadır. Eksteme lateral veya posterior yolla takılmış disk protezlerinin sonuçları ile ilgili az sayıda makale bulunmaktadır.

Sonuç: Posterior yolla takılan disk protezlerinde komplikasyon oranları yüksektir. Kullanım endikasyonları ve uzun dönem sonuçları için vaka sayıları yetersizdir.

Introduction and Objective: Lumbar disc prosthesis is a method developed as an alternative to fusion surgery. Since adjacent segment disease is one of the biggest problems of fusion surgery, alternative methods have always been tried. In this study, we discussed lumbar disc prosthesis and its complications in the light of the literature.

Materials and Methods: A 38-year-old woman was admitted to our clinic with complaints of pain in the anterolateral aspect of the right hip and thigh, which had been present for 6-7 months and recently intensified. She underwent discectomy and total intervertebral disc prosthesis was implanted at the L4-L5 level due to an idante disc in the canal- After 1 year of follow-up, the patient complained of severe left leg pain and the prosthesis was observed to be dislocated into the canal. The material was removed with reoperation.

Results: Disc prostheses implanted through the posterior route cause fewer complications than the anterior and extremity lateral route. When the literature is reviewed, confusion about lumbar disc prosthesis still persists. However, there are many publications on the superiority of the anterior approach. There are few articles on the results of disc prostheses implanted via the extensor lateral or posterior route.

Conclusion: Posteriorly implanted disc prostheses have high complication rates. The number of cases is insufficient for indications for use and long-term results.
