ABSTRACT

INTRODUCTION
Teeth with wide apical foramina, external resorption, and severe coronal loss present complex endodontic and restorative challenges. While MTA is the gold standard for apexification, the optimal timing for post placement remains a subject of debate.

CASE DESCRIPTION
A 25-year-old male presented with pain and extensive destruction of the maxillary left central incisor. Radiographs revealed a recurrent periapical lesion, apical resorption, and an enlarged foramen. Following retreatment and two-week calcium hydroxide therapy, the canal was prepared to ISO size 80. Irrigation included 2.5% sodium hypochlorite and 17% EDTA with ultrasonic activation (DTE, China). An apical barrier was created using mineral trioxide aggregate (Angelus MTA, Brazil). Due to severe tissue loss, a fiber-reinforced post (Cyctec Fiber Post, China) was placed in the same session immediately after MTA application. The tooth was restored with a monolithic zirconia crown. At one-year follow-up, the patient was asymptomatic with significant radiographic bone healing.

DISCUSSION
Standard protocols often delay post placement to ensure MTA setting. However, immediate placement prevents potential coronal leakage in severely broken-down teeth. This approach maintains the apical seal while providing early structural reinforcement.

CONCLUSION/CLINICAL SIGNIFICANCE
Single-visit apexification with immediate post placement is a viable treatment for teeth with wide apices and extensive destruction, facilitating prompt definitive restoration and successful healing.
