Bu olgu sunumu, gebeliğin ikinci trimesterinde akut karın ağrısının nadir bir nedeni olan spontan tubal torsiyonun tanı ve yönetimini ele almaktadır. 26 haftalık gebelikte ani başlayan şiddetli sağ alt kadran ağrısı ile başvuran hastada ultrasonografi ve manyetik rezonans görüntüleme, overlerin normal olduğunu ancak sağ fallop tüpünde torsiyon düşündüren değişiklikler bulunduğunu ortaya koydu. Laparotomi sırasında sağ tüpün kendi etrafında iki tam tur dönerek nekroze olduğu saptandı ve salpenjektomi uygulandı. Postoperatif seyir sorunsuz geçti ve gebelik sağlıklı şekilde devam etti. Bu vaka, gebelikte nadir görülen ancak hızlı cerrahi müdahale gerektiren tubal torsiyonun, akut karın ayırıcı tanısında mutlaka göz önünde bulundurulması gerektiğini göstermektedir.
This case report discusses the diagnosis and management of spontaneous tubal torsion, a rare cause of acute abdomen during the second trimester of pregnancy. A 26-week pregnant woman presented with sudden-onset severe right lower quadrant pain. Ultrasonography and magnetic resonance imaging showed normal ovaries but changes suggestive of torsion in the right fallopian tube. Laparotomy revealed that the tube had twisted around itself twice and become necrotic, leading to salpingectomy. The postoperative course was uneventful, and the pregnancy progressed without complications. This case emphasizes that although rare, tubal torsion should be considered in the differential diagnosis of acute abdomen during pregnancy, as prompt surgical intervention is essential.
