Özet

Objective: Lateral ventricular arachnoid cysts are rare, and evidence guiding their management remains limited. This study evaluated the clinical characteristics, radiological findings, and management outcomes of patients with lateral ventricular arachnoid cysts.

Methods: We retrospectively reviewed patients with lateral ventricular arachnoid cysts evaluated and managed at the Department of Neurosurgery, Akdeniz University Faculty of Medicine, between January 2016 and December 2025. Demographic characteristics, presenting complaints, imaging findings, follow-up duration, treatment approaches, and postoperative complications were assessed.

Results: Among 266 patients with arachnoid cysts, 13 (4.9%) had intraventricular cysts, all involving the lateral ventricles. Seven patients were female and six were male. The mean age at the last clinical visit was 17 years (range, 3–65 years). Follow-up ranged from 3 months to 18 years among patients with repeated visits. Headache was the most common presenting complaint (n=5), followed by trauma-related presentations (n=3), visual disturbances (n=2), seizure (n=1), and cranial deformity (n=1). One cyst was diagnosed prenatally. Eight cysts were right-sided and five were left-sided. Two extended into the interhemispheric region and one into the third ventricle. Ventricular dilatation was present in three patients, and corpus callosum agenesis in two. Seven patients were managed conservatively, with no spontaneous cyst regression observed. Six underwent surgery: three received cyst fenestration and three underwent cystoperitoneal shunt placement. No postoperative complications were documented.

Conclusion: Lateral ventricular arachnoid cysts exhibit heterogeneous clinical and radiological features. In this series, seven patients were managed conservatively, while six underwent surgical intervention without documented postoperative complications. Management should be individualized according to clinical presentation and radiological findings. Larger studies are needed to clarify surgical indications and long-term outcomes.
